Accelerator response and gear change

1) the first 2" of accelerator push does nothing at all, only responding after about 3" down. I assume a cable needs tightening or replacing, if this isn't normal?

2) when i accelerate faster than what you'd term "normal", the gearbox whines when it changes gear. Am i simply being to hard on it?

3) Why is the 2nd gear after the 3rd gear, yet surely you're meant to put it in 2 before 3? (auto transmission)

1) Mercedes from that era should have about 0.5-1 inch of play (by that I mean the pedal is stiff but it doesn't rev) in the accelerator before it revs - its a characteristic of their cars from the era (you'll find a Rolls/Bentley is exactly the same). 2"-3" does sound excessive and needs adjusting.
